Relational trauma results from experiences that are perceived as overwhelming and dangerous and are defined as a violation of trust and betrayal by someone depended on for safety and security.I refer to my treatment approach when working with these couples as relational triage. In these cases, while I most often see other presenting issues needing clinical attention, in the aftermath of the discovery of sex addiction or sexual betrayal the partner's acute trauma must be addressed at the outset of treatment. If the initial focus of couples therapy is on anything other than partner trauma, her symptoms are exacerbated which ultimately escalates conflict for the couple. Until the partner feels validated by the person who has betrayed her and sees that he can consistently demonstrate empathy for not only the original betrayal but also the subsequent triggers and trauma, relational repair cannot be achieved. Relational triage is about first assessing what stage a couple is at in their healing and then identifying and providing education and specific tools o help them heal from relational trauma. Relational triage starts with addressing partner trauma and includes education on triggers, survival responses, co-regulation and specific communication techniques i.e. IMAGO dialogue to foster healthy communication and empathy building.Marnie Breecker, MFT, CSAT, CCPS, is the Founder and Clinical Director the Center for Relational Healing CRH in Los Angeles.
